Practitioner Dispensary Guidelines

Here’s where you’ll learn how you can order custom formulas on behalf of your clients/patients. To use the program, please read this page in its entirety and sign the associated form (linked below).

How it Works:

Once you have been approved to use this ordering system, you can come to the practitioner’s portal here on our website and place an order for your client/patient. Once your order is placed, you will receive a confirmation that your order was received, and we will take it from there! We will contact your client to arrange for payment and pick-up/shipping. You’ll find information about our inventory, pricing, FAQs, etc. in the practitioner’s portal and in the “apothecary” section of our website.

How to Place Orders

When you’re ready to place an order, you’ll head to the practitioner’s portal (linked in the footer of every page of our site) and complete the HIPPA compliant order form. Here’s what you’ll need to include in this form:

For each formula you order, please provide the following information:  

1.⠀⁠Client’s Information

  • For new clients, please provide their first and last name, phone number, and email address. Your client will need to fill out a waiver, which we will send to them. You can also find the waiver in your client portal.

  • For clients who we have formulated for within the past 2 years, you can simply provide their first and last name.

2. Name of the Formula

  • Each custom formula gets a name (e.g. Sleep Well Tea, Cramp-Away Tincture) so that we can easily look up the herbs in any given formula.

3. Type of Formula

We offer:

  • Teas

  • Alcohol Extracts (Tinctures)

  • Powders (some powder formulas will incur an $8 powdering fee)

  • Topical Oils (herb infused extra virgin olive oils, plus carrier oils like castor and jojoba)

  • Topical Liniments (topical consisting of a mixture of herbal oils and tincture)

  • We have a limited number of Glycerites available

4. Amount

  • We sell extracts, topical oils, and liniments by the fluid ounce, typically in 1, 2, 4, 8, or 16 ounce sizes, as those are the sizes of our bottles.

  • We sell loose herbs, tea blends, and powders by the ounce.

  • Our custom tea blends are $9/ounce. Custom extract blends are $19/ounce. Custom oil blends or liniments are $19/ounce. Custom salves and ointments are $19/ounce.

  • Bulk Herb Discounts: For each individual preparation, we offer a 15% discount after 4 ounces, a 20% discount after 8 ounces, and a 25% discount after 16 ounces.

  • Order Minimum: 1 ounce/fluid ounce is the minimum amount you can order of any individual formula, with the exception of low dose extracts and higher priced herbs such as American ginseng.

5. Formula Components

  • Provide a list of the herbs in the formula with the exact amounts (in milliliters or ounces) of each herb.

    • Extracts, oils, and liniments are measured in milliliters. Teas and powders are measured in ounces

    • For amounts in milliliters, please use whole numbers (no decimals)

  • Botanical names are encouraged, especially for herbs with ubiquitous/multiple common names.

  • Specify the part of plant, if possible. There are a handful of herbs that we carry different in forms (e.g. hawthorn leaf and flower and hawthorn berry, or nettle leaf, nettle root, and nettle seed).

  • We can combine up to 10 herbs in either a custom extract formula, tea formula, topical oil, or topical liniment formula.

6. Dosage Instructions

  • Provide your own dosing instructions.

  • Provide your own preparation instructions if they are different from ours (see below).

Examples

Herbal Tea Order Example:

4 ounces “Herbal Coffee” Tea for Jane Doe

  • 1.2 oz burdock root

  • 1.2 oz dandelion root

  • 1.2 oz chicory root

  • .4 oz cinnamon bark (c. burmannii)

Instructions: Drink 2 cups/day

In the above example, the name of the formula, amount, client’s full name, formula type, amounts, and instructions are all provided. The latin name of cinnamon is clarified since we carry both cinnamomum burmanii and cinnamomum verum.

We ask that you send in orders with the exact amounts of each herb, rather than the parts or percentages of each herb. In the examples below, you can see what this formula would be in parts and percentages. Please determine what the amounts of each herb would be, rather than sending in a formula like the one’s below.  

4 ounces “Herbal Coffee” Tea

  • 3 parts burdock root

  • 3 parts dandelion root

  • 3 parts chicory root

  • 1 part cinnamon bark

4 ounces “Herbal Coffee” Tea

  • 30% burdock root

  • 30% dandelion root

  • 30% chicory root

  • 10% cinnamon bark

Herbal Tincture Order Example:

4 ounces (120mL) “Immune Support” Tincture for Rose Wood

  • 48 mL skullcap

  • 36 mL albizia julibrissin bark

  • 24 mL passionflower

  • 12 mL rose

60 drops 3x/day

In this example, the extract would contain 4 parts (40%) skullcap, 3 parts (30%) mimosa bark, 2 parts (20%) passionflower, and 1 part (10%) rose. The botanical name was provided for the mimosa to clarify that this is not mimosa pudica, and the plant part was provided because we carry both mimosa bark and mimosa flower.

Formulation Assistance

If you would like our assistance creating a formula for a patient/client, we are happy to do so. We charge a formulation fee of $35 per formula.  

Refills

If you are ordering a refill for your client/patient, you can use the practitioner portal to place an order.

If your client would like to order a refill of a formula, they can use the order button on the contact page of our website, OR call us with their name, the name of their formula, and the desired quantity. We will call/email your client back to confirm we received the order and arrange payment and pick-up time. Wait times vary when placing an order in- store, so we highly recommend that your client/patient order their formulas ahead of time.

Our standard preparation instructions:

Herbal Tea Preparation Instructions:

  • Leaf/Flower Tea Instructions: Use 1 tablespoon of tea per cup of water. Place herbs in a vessel and pour boiling water over them. Cover and steep for at least 1 hour, up to overnight. Strain herbs and enjoy.

  • Root Tea Instructions: Use 1 teaspoon of tea per cup of water. Place herbs in a pot with water and bring to a boil. Simmer for 15-30 minutes. Remove from heat and steep for at least 1 hour, up to overnight. Strain herbs and enjoy.

Herbal Tincture Instructions:

Take the tincture orally either directly in the mouth or diluted in ~one ounce of water.
